I established a new medical practice in January 2015. When is the first provisional tax payment due?

In your personal name or as a company?

I am the sole proprietor of the practice. Does it have to be considered as a company or part of my personal income? That will probably determine the answer. My personal tax for 2015 has been filed since I was employed previously and earned a salary.

This will form part of your personal income. Your personal return for 2015 is only open to be filed in July 2015 so this can not be the case. Your provisional return for the 2015 year is due 27th February 2015 and must be settled by then as well.
