Since I have already uploaded my material, could you advise on the best way to format this schedule so it satisfies the auditor before the April 7th deadline? |

I have prepared and attached a template for you to use. The first row is an example please delete this and complete your own first row. I will briefly explain what each column entails below : Taxpayer Name: State your name Income Tax Number : State your income tax number Tax Year : State the tax year SARS is querying eg 2025 Reference: Sequence number of the receipts you have presented to SARS ie first receipt would be sequence 1. Date Paid : Date that you paid for the receipt provided Service Provider: Name of the service provider eg Click, Dischem or Dr Who Practice/Prescription name : Prescription number starts with RXNO this is found on the receipts you have. You may state the doctor's practice number where applicable. Amount paid by taxpayer: what you paid Amount paid by medical aid : state the amount the medical aid if any Supporting documents : state yes if you have attached the receipts and no if no receipts.
