Which bank details do I fill into my IRP5/IT3?

I changed banks last year December, so I am confused on which bank details I must fill in on my IRP5. Do I go with the new bank details, the older bank details, or both? Some help would be appreciated...

I am trying to do the right thing by filing my return, but what I am getting is confusing. My total income is (3601- R555 184;3805- R75 694; 3810- R72 830; 3616- R8 175;3809- R500) = total 712 383.

When I complete my return and do a mock lation it shows I owe R1225,64. My medical aid tax credit (4116) is R9 216 - for 4 people. My question is why am I being levied R 217 093.20 (for R712 383) without any deductions especially for medcial aid contributions? There is rebate of R20 656 de...

Last year I worked for the education in the Eastern Cape, the grade 12 marking project. 25%( R1400 ) of my salary was deducted and reportedly paid to the Receiver of Revenue. I went on maternity leave for a full four months mind February 2012 - Mid June 2012. I was paid for the first two months and then unpaid for the remaining two months. In my IRP5 : Periods in year of assessment is 12 and no. of periods worked is 12. I believe this is incorrect. Do I correct this and say I worked 8 months? What is the correct information to put in this case?

I am employed by the government. I do not receive medical aid subsidy. I do however get a tax rebate. Do I still qualify for the tax credit? If so, how do I go about claiming it? Also, does the tax credit mean that we can no longer claim medical expenses not covered by the medical aid for which we pay cash?

How is code 4008 calculated for 4 members under medical scheme? I understand there is a maximum amount but R3783 doesn't seem right?

I earn a basic and my commission is more than 50% of my annual basic. I sell luxury automobiles for a living.

I see on my IRP5 under "Income received "an amount of R65 000 "Use of Motor Vehicle" (PAYE)

Is there a specefic place where I insert this amount on my tax return?
