I got a letter from my medical aid saying "the amount not covered by scheme is R1092". Does this mean I owe SARS?

No, it does not. You need to fill in this amount in your tax return under the Medical Aid section "Medical expenses not recovered from your medical scheme" (code 4020).

It will be used in the formula to calculate your overall Medical Tax Credit to be deducted off your total tax payable.
