I have a IT3(b) tax certificate from the bank , they gave me a income source code 4201 , please tell me where it is on the ITR12 ?

This is for local interest income. When you open your tax return, you will be asked a series of questions which are used to generate your Tax Return. In the first section entitled "Standard" there will be the following question:

"Did you receive income from interest (local or foreign) and /or taxable foreign dividends?"

You need to answer this "yes".

In the tax return (ITR12), you need to fill in this amount from your IT3(b) certificate under "Local Interest income".
