As a provisional tax payer what are the periods (months) that I need to consider in order to calculate my tax submission for 28 Feb 2016?

You need to calculate the tax based on your estimated taxable income for the entire tax year (i.e. 1 March 2015 - 28 Feb 2016) and then reduce the tax by your 1st provisional tax payment you may have made in August 2015.
