How do I obtain IRP5 form from previous employer?

I called SARS helping line yesterday to ask about my tax number only to find out that an agent I have worked for last year early for only a month has registered me. So now I have been told to submit an IRP5 form at SARS but where do I get this form now cause I've only worked with that agent last year June and that was it.

She needs to give you the IRP5 in order to submit it to SARS.
