I am 70 years old,
Question 1 : I do casual jobs for 2 companies - I invoice them and they pay me at the end of the month - my approx yearly total salary is about R170k, depending on the times their call me in. They do not take PAYE off. I also get about R10k of interest on a small investment (but I am married in comm of property). Medical Aid contrib are about R23k which we pay out of our pocket for a Discovery hospital plan but we get a medical tax certificate for. Question 2 : The one company wants to deduct PAYE - how will this impact me if I am a Provisional Taxpayer?
Question 3 : One of the companies has said that they won't need me for the next month or two but maybe later - how will this impact me - will I still a Provisional taxpayer? They paid me about R50k pa, so my yearly salary would then be about R130k

Please answer the below questions to confirm whether or not you are a provisional tax payer:
Am I a Provisional taxpayer?

If the company deducts PAYE, when you complete your provisional tax return, you would still include the taxable income earned from this company - however, you would also include the related PAYE, so the result would be no provisional tax payment for this company. However, you would still pay provisional tax on earnings from the other company.

Please do read our blog on What is Provisional Tax? How and when? for further clarification.

Even if you don't work every month, as long as you are earning above the tax threshold which is currently R114,800 per annum for your age, you will need to pay provisional tax.

Yes, you would be a provisional taxpayer. You do not earn a salary and have tax deducted every month and you are above the tax threshold.

Please make use of our a href="https://www.taxtim.com/za/calculators/income-tax">SARS income tax calculator to see the tax you would pay on your earnings for the year.
