How do I create a username and password for myself?

On SARS eFiling (www.sarsefiling.co.za) you will need to click Register first, then click 'For individuals' and register for an account with eFiling. They will send you your login name once registration has gone through. You can make up whatever password you like, but must follow their guidelines as explained there.
