My son is over 21 years and still my dependent , as he is still going to university . He has fallen off my existing medical aid as a dependent due to his age . I have now arranged a new medical aid for him and I personally contribute with my after tax income . How do I claim this expenses as he is not working?

Yes, because he is a dependent of yours, you should be able claim the medical aid credit for him.
Just complete the medical aid section in your tax return with details from the Medical Aid Tax Certificate and include him as a dependent. If SARS does request supporting documentation, you need to be able to justify that he is a dependent and you therefore pay his medical aid on his behalf.
