how to get IT34 From SASSA?

I have been asked to send IT34 by my medical scheme in order to processed a document, so how can I get hold of that ITA34?

You need to go to your eFiling profile, then click under the "returns" tab the "SARS correspondence" then click, "request historic IT notices"
