Can't get old IPR5 from previous employer

What should I do if I can't get my old IRP5 from my previous employer, SARS is owing me R6000

You need to go to your local police station and Swear an affidavit giving the reasons why you are unable to obtain the IRP5. You could also submit bank statements and/or payslips to SARS if possible as proof of your earnings for the period in question.
