For Provisional tax that's due now, do I enter medical credit for the year as per SARS instructions and then half it or not? Same question for medical expenses, do I add up medical expenses for 6 months (March to August 2015) and then multiply by 2 for estimate for one year or not to multiply and submit actual?

You need to enter both these amount based on the full year i.e 12 months.
The Return will automatically calculate the tax and then halve it so that you pay only 50% now in August.

See further clarification at the link below - specifically, look at the section around medical expenses because it is a very complicated formula that is applied to work out the Additional Medical Tax Credit.
